Terms of Use

Terms for the FormByForm country gateway. Version dated 2026-08-24.

1. Operator and scope

  1. These Terms govern the global country gateway available at formbyform.com.
  2. The operator is Mateusz Król, address: ul. Żurawinowa 7, 42-271 Częstochowa, Polska, Polish tax ID (NIP) 5732923161, REGON 387632869 (the "Operator").
  3. Contact: [email protected]. Polish and English can be used.
  4. The Terms are available free of charge at /terms in a form that can be displayed, saved and printed.
  5. These Terms apply only to the global gateway. A country service reached through an outbound link is governed by the terms and privacy policy published on that service's domain.

2. Purpose of the gateway

FormByForm provides general information about the global brand, displays available and planned country services, and lets a visitor choose an available country explicitly.

The gateway is not a shared form catalogue. It does not provide a form editor, document generation, document submission, an account, a contact form, ratings, comments, payment or an automatic country redirect. A planned country card is informational and is not a promise of a launch date.

3. Country links

An available country card is an outbound link. Choosing Poland, for example, opens wniosek-wzor.pl. The destination is a separate country service. Its forms, content, availability, legal terms and privacy choices apply only after the visitor opens that destination.

The Operator aims to keep links accurate but cannot guarantee that a destination will be continuously available. The visitor should verify the address and the country context before using a linked service.

4. No official status or advice

Neither FormByForm nor the global gateway is a government authority, public office or official submission channel. It is not affiliated with, endorsed by or acting on behalf of a government authority unless a page expressly states otherwise.

Gateway information is general navigation information, not legal, tax, accounting or official advice. The gateway does not select a jurisdiction for the visitor and does not submit a document or application.

5. Technical requirements and acceptable use

An up-to-date web browser and internet access are required. The Operator may temporarily limit the gateway for maintenance, security, provider failure or events outside reasonable control.

Visitors must not attack the gateway, bypass safeguards, introduce harmful code, overload the service, scrape it in a way that disrupts availability, impersonate the Operator or use the brand or content unlawfully.

6. Intellectual property

The brand, original text, design and software belong to the Operator or applicable licensors. Visitors may view and save the public legal documents for personal reference. These Terms do not grant permission to copy the branding, present the gateway as an official public service or operate a confusingly similar service.

Country names and references to public institutions are descriptive and do not imply sponsorship or endorsement.

7. Privacy

The gateway's limited processing is described in the Privacy Policy. The FormByForm application does not accept form data or save drafts. When a separate FormByForm Google Analytics stream is configured, the application offers an analytics choice and does not contact Google before consent. It does not use Google Tag Manager, advertising services or cross-domain measurement. A network security provider may use a strictly necessary anti-abuse cookie only under the conditions disclosed in the Privacy Policy. Opening a country service starts a separate request governed by that destination's privacy policy.

8. Complaints and contact

Questions or complaints about the gateway can be sent to [email protected]. A complaint should identify the issue, date, requested outcome and reply address. The Operator responds without undue delay and no later than 14 days after receiving a complete complaint. Mandatory consumer and court rights remain unaffected.

9. Liability

The Operator is responsible as required by applicable law. Nothing in these Terms excludes liability that cannot legally be excluded or limits mandatory consumer rights.

The Operator is not responsible for a visitor choosing the wrong country, for actions taken on a destination after leaving the gateway, or for a destination provider's independent content where the Operator does not control it. This does not limit responsibility for the Operator's own selection and presentation of links.

10. Changes and final provisions

The Operator may update these Terms for an important reason such as a change in law, the gateway's scope, security, technology or operator details. The current version and effective date are published at /terms; a change does not operate retroactively against a completed use.

Polish law applies. This choice does not deprive a consumer of mandatory protection in their country of habitual residence. If one provision is invalid, the remaining provisions continue to apply to the extent permitted by law.
